Mrs. J. is a 78 year old widow with severe osteoporosis and arthritis. She uses a walker and is unable to go out alone. Volunteers take Mrs. J. to her medical and dental appointments at least twice a month and deliver groceries to her on a weekly basis.
The primary goal of Senior Services is to help seniors in the community maintain their independence. We offer Case Management, Community Outreach, and Senior Nutrition.
Our case managers are highly skilled geriatric care professionals with a heart for the needs of our community's elderly. We meet with clients in their homes to determine what services they need in order to live independently. Whenever possible, we involve family members in developing a care plan that meets both the physical and emotional needs of every client. We provide referrals to community resources, make arrangements for in-home services, and work cooperatively with care providers. Ongoing monitoring is provided.
Through Community Outreach, volunteers provide seniors with escorted transportation to and from their medical appointments. Clients also rely on us for grocery shopping and other errands, and volunteers visit clients in their homes.
The Senior Nutrition program, located at the Mountain View Senior Center, provides subsidized lunches every weekday to clients over the age of 60. In addition to nutrition, these lunches offer a time for socializing and education. We hold regular dances, exercise programs and blood pressure screening, as well as cooking/nutrition classes and seminars on current issues that are important to seniors.
CSA Caseworkers are available on the first Friday of each month at the Mountain View Senior Center from 10:00-11:00 AM.
On the Fourth Monday of each month, CSA caseworkers will be at the Los Altos Senior Center from 1:00-3:00PM.
